The following slides briefly illustrate the creation of a teaching or module/course survey instrument.
Access Control
Registration is not required to access this web application. Rather, enter your institutional email address, the security code and tap or click Continue. You will then receive an email with an eight-digit access code. This process is repeated each time you visit or refresh this web application.

Enter the access code and tap or click OK. If the email address displayed is incorrect tap or click Cancel, correct your email address and try again.

Each time you enter the application you need to enter or review your biographical data.
Survey Creation
Tap or click the + button at the bottom right of the screen to create a survey.
Survey Creation
Select the type of survey you would like to create. These include evaluation of teaching or course/module evaluation. Two types of teaching surveys are available: one with scalar items (core and optional) and open-ended questions, and another with open-ended questions only.

Complete three tasks to create a survey: 1. Provide module/course information that includes the purpose of the survey; 2. Select additional scalar items; and 3. Review and approve the survey. A survey is only saved once it is approved. Click or tap the red button to exit survey creation tool.

Survey Creation Additional Items
The additional items are grouped into categories. Click or tap a category to display the related items that you can select. For each survey type you can select a maximum number of items that is shown in the header.

Once you have saved the Module/Course information and the additional items, you will be able to review and save the survey.

Review the survey. Return to the menu if you wish to make changes, or save and approve the survey. After saving, the creation tool will close and your survey will be added to the survey list.

A list of all the survey you create is available. Click or tap each item to access the related menu items.

The Edit survey option is available until a student response is posted. Students can access the survey after you open surveys for distribution as described in an email sent to you. Once students submit their responses you will be able review the reports. You are expected to add a comment to the student report.
